AI in Healthcare Is Promising, But Hospitals Cannot Ignore the Security Risks

Artificial intelligence is becoming one of the biggest talking points in healthcare, and it is not hard to see why. Hospitals and health systems are under constant pressure to do more with limited staff, rising patient expectations, tighter budgets, and growing operational complexity. In that kind of environment, AI naturally looks appealing.

The excitement is not just based on theory either. Early results are already showing that AI can help in meaningful ways, from administrative tasks such as coding and workflow support to more advanced clinical use cases that touch patient care directly. That is why so many healthcare organisations are moving quickly to explore where AI fits into their long-term strategy.

But there is another side to this conversation that deserves just as much attention. As hospitals bring in more AI tools and connected systems, they are also introducing new security risks. In other words, the opportunity is real, but so is the exposure.

AI Adoption Cannot Be Treated Like a Casual IT Upgrade

One of the clearest lessons emerging from healthcare AI adoption is that these systems cannot be treated like simple plug-and-play tools. They affect workflows, data access, clinical operations, governance, and risk management all at once. That means implementation needs to be handled with much more structure than a typical software rollout.

At Akron Children's Hospital, that process is approached in a highly organised way. According to Deepesh Randeri, the hospital's chief information security officer and vice president of information security and infrastructure, every new technology must go through a defined approval path involving governance, leadership review, cost and return considerations, and, most importantly, security.

That kind of structure matters because AI is not just another shiny tool being added to the environment. It has the potential to influence decisions, interact with sensitive systems, and touch protected health data. If that happens without strong oversight, the risks multiply quickly.

Due Diligence Has to Happen Before Anything Goes Live

A major part of that structured approach is strict due diligence. At Akron Children's, new vendors and systems are not simply allowed in because they look useful or because a department wants them. They must go through a rigorous vetting process before they can be connected or implemented.

That may sound strict, but in healthcare, it makes complete sense. Hospitals operate in a highly sensitive environment where even one weak point can create serious consequences. A rushed rollout might save time upfront, but it can create security issues, operational disruption, or compliance trouble later.

This is especially important with AI, because these systems can sometimes feel more abstract than traditional hardware or software. People may focus on the promised outcomes while underestimating the importance of verifying how the system actually behaves, what data it touches, and whether it meets the security standards the organisation expects.

The Harder Problem Often Comes After Approval

Many healthcare organisations are getting better at front-end AI governance. They have committees, approval processes, and evaluation discussions before implementation begins. That is progress. But according to Randeri, the real challenge often starts after the system has already been approved and deployed.

This is where many organisations can lose control.

It is one thing to approve a system on paper. It is another to verify that the version implemented is exactly the one that was reviewed, that the promised guardrails were actually applied, and that the tool continues behaving as expected once it becomes part of daily operations.

That is where back-end oversight becomes critical. Without ongoing monitoring, governance can become little more than a launch-stage exercise. And with AI, that is not enough.

Security Is Not Just About Access, It Is Also About Integrity

When people talk about AI security, they often think first about breaches, attackers, or unauthorised access. Those are still major concerns, of course. But in healthcare, AI security also has another layer: integrity.

Hospitals need to know that the system deployed is the system that was approved. They need to know that no unexpected bias has been introduced. They need to know that the controls discussed during governance meetings are not just theoretical promises but actual protections in place.

This is what makes AI oversight more demanding than many traditional IT projects. It is not only about keeping bad actors out. It is also about making sure the system itself remains trustworthy, aligned with policy, and safe for the environment in which it operates.

Leadership Still Sets the Tone

Another important point is that AI security cannot be treated as a niche issue owned only by cybersecurity teams. Leadership has to make it clear that security is part of everyone's responsibility.

Randeri's view is that strong tone from the top matters because it helps ensure executives understand the implications of adopting new technologies. That awareness is important. When leadership takes security seriously, governance becomes stronger, departments are less likely to bypass controls, and the organisation is better positioned to implement innovation responsibly.

Healthcare has seen this pattern before with previous waves of technology. The tools may change, but the principle does not. New systems always bring new possibilities, but they also bring new ways for things to go wrong.

The Old Security Model Is No Longer Enough

There is also a broader shift happening in how healthcare organisations think about security itself. The older model was often based on a strong perimeter, protecting the boundary and assuming that what was inside the network was more trusted. That model is no longer enough.

Today, hospitals are working in a much more distributed environment involving cloud systems, identities, third-party services, remote access, and increasingly intelligent platforms. In that world, security has to be built around identity, access control, monitoring, and layered protections rather than just a digital moat around the organisation.

AI fits directly into this challenge. Just as phishing became one of the easiest ways for attackers to gain entry, poorly governed AI tools can create new paths for risk if they are not properly controlled.

The Risk Is Not Only in the Cloud

It is also important not to assume that danger comes only from third-party cloud platforms. AI-related risks can exist in technologies running inside the healthcare organisation itself.

Whether a system is external or on premises, the same rule applies: if the right controls are not in place around the technology, the people using it, and the processes surrounding it, the organisation is left exposed.

That is a useful reminder because many technology discussions become too focused on where the system lives rather than how well it is governed. Location matters, but governance matters more.

Final Thoughts

AI in healthcare is moving from experimentation to real-world implementation, and the promise behind it is becoming harder to ignore. Hospitals can already see how these tools may improve efficiency, support staff, and strengthen certain clinical and administrative processes.

But none of that removes the need for discipline. In fact, it makes discipline even more important.

The real challenge is not simply bringing AI into a hospital. It is bringing it in with the right governance, the right due diligence, the right monitoring, and the right security mindset from beginning to end.

Healthcare organisations that get this balance right will be in a much stronger position to benefit from AI without opening the door to avoidable risk. And in a sector where trust, safety, and continuity matter so much, that balance may be the most important part of all.
